I think there were multiple ways they could be made, but a lot of them changed - or WERE changed - by using magic or relics they weren’t capable of wielding. It… twisted them, changed them. Turned them into mindless monsters bent purely on rage and destruction. Sometimes it was voluntary and others… not so much.
They’re typically too dangerous to try and take down with a single fighter, better to apply pack tactics in a fight against it. Split it’s focus. They can hit hard and they’re smarter than your typical beast. They might not possess their human consciousness anymore - most of them, anyway - but they can still use more strategy than most. They’re not to be underestimated.
